What the Contact Form Collects
The contact form asks for your first and last name, your email address, and your message. Your phone number and your reason for contacting us are optional. Nothing else is requested.
Please do not use the form to send detailed medical history, diagnoses, images, or records. Email is not a secure channel. Call us instead and we will collect clinical information through a protected route.
What Happens to a Submission
A submission is composed into an email and delivered to the clinic inbox through Amazon Simple Email Service, which acts as our mail provider. It is not written to a database on this website, and this website has no patient portal or account system.
Once the message reaches the clinic inbox it is handled like any other patient enquiry, under the practices described in our Notice of Privacy Practices.
Analytics and Cookies
This site uses Google Analytics to count visits and understand which pages people read. Google Analytics sets cookies in your browser and records information such as the pages you view, approximate location derived from your IP address, referring site, and device and browser type.
We use this only in aggregate, to decide what to publish and to fix what is not working. We do not combine analytics data with patient records, and we do not use it to identify individual visitors.
